ShadowOwl: seems like you dont have to be dead to ascend. As is oft repeated, there are no hard and fast rules to what ascension requires.
As made plain by my earlier comments in the thread, I agree with Gothos regarding this topic, but let me try and clear some stuff up.
1) TTT is the race of toblakai, much like Human is... human, and then you have different locations/looks, in example the difference between a human from Darujistahn(sp) and the seven cities.
2) Yes, it is mentioned some powerful TTT's become a warren unto themselves, and Pearl notices a TTT warren on the ship, after the malazans have it, which is after Karsa kills some Edur, and Rhulad's sorcerer brother on it.
3) Avowed's of the Crimson Guard seem to be able to, using their ritual (which seems to me somewhat like the Imass one) and their willpower, heal themselves quickly, fight faster/better. IB fought 6 or 7 half-ascended TTT for a little while, before Ruin walked up naked (which I'm sure scared them) and sliced them up. IB mentions that he will be fine unless/until his concentration wanes. As a note, his mage was helping confuse the TTT for part of the fight.
4) CG did offer the sword to Karsa after he killed Rhulad. He did say it was meant for him all along. Truth? Probably not. Does it really matter at this point? Although it is noted, in Trull's conversation with someone, Onrack i think, that he is concerned because Rhulad, and his patron, were only seeing to the short term, which would mean they would be discarded, or disposed of, after their near goals were reached. (KCCM Invasion Anyone?)
5) It is a tragic series, but an honest one. There is a price for every victory the 'good' side has, and that is as it should be. Who will be standing at the end? I would think very few people, and even fewer who are the 'current' powers, as the CG goes about getting his revenge.
