This comic really helps explain what went wrong in the "Dark Future."

Essentially, Sylar killed Claire and became invulnerable. Hiro tried to kill Sylar just before Sylar went nuclear, having gained Ted's power. Hiro failed, Sylar went boom, NYC went bye bye, Nathan became the nation's hero and the Heroes became hunted criminals.

Future-Hiro tried to avert the Sylar-boom by telling Peter to save Claire. However it seems like Peter will take Sylar's place as the bomb, thus rendering Future-Hiro's plans unsuccessful.
