Biden also has had a LIFELONG struggle with speaking and stammering...so any comments on his speech are silly. Is he too old for the position? Seriously though, the main concern isn't risk of dying, but cognitive decline and energy level.
Unfortunately biology does not necessarily reward 'virtue' or punish 'vice'; plenty of obese people who eat like Trump does and don't exercise don't get heart attacks. But
Trump's diet, obesity, and lack of exercise make him more likely to die than he otherwise would be, so you're probably right that (given what little we know) he's more likely to die or have a debilitating stroke than Biden is. However, most of Trump's voters don't care about that, and don't believe it will happen. Most probably because they believe he's favored by Go*. Potential Biden voters OTOH are generally more reality-based, and many Biden voters are voting against Trump more than anything... but too many also apparently wouldn't vote for Kamala Harris.
