D'rek as a god seems to embody the concept of 'rot' or the passage of time as erosion. Worms being a creature that basically survive on and assist that process have a symbolic link. D'rek may not be an actual 'worm' in the sense that Fener or the Wolves are ascended beasts.
It's been suggested at least once, i think by Banaschar in TB, that D'rek may actually be an Elder God or elemental force. No one really knows how long D'rek's been around and the aspect would suggest it's even older than the Wolves.

I have a vague recollection about D'rek being present in Banaschar as remarked upon by Curdle and Telorast in I think RG. Although what this means is beyond me.

Yeah, it seems that D'rek is dwelling inside Banaschar's guts because he's his last priest.(Tayschrenn having given up).

I just so happened to come upon this paragraph on my RG reread [from Banaschar's POV]:
The Worm of Autumn stirred in his gut; an immortal parasite and he its mortal host. The goddess was within him once more, after all these years. Again, no surprise. After all, I'm the only one left. Yet D'rek remained as no more than a presence, a faint taste on his tongue. There had been no battle of wills; but he knew it would come. The goddess needed him and sooner or later she would reach out and close a cold fist about his soul.
[p. 504 in the trade paperback]
Yah something similar to Poliel needing a mortal host in TB, even tho she tended to kill those hosts eventually.

It's said that D'rek can be both a he or a she depending on his/her followers. And yeah I also understood that he/she killed his/her priests because they had allied with the CG.
D'rek's priesthood decided that they wanted to ally with the CG and their prayers were pushing D'rek towards that alliance. D'rek didn't feel like it so it killed them all.

quick note: in GoTM, Mammot was a High Priest of D'Rek
Yep, and someone theorizes that if Raest dominates Mammot the Tyrant may have a way to then dominate D'rek. Debateable if D'rek is actually as powerful as it seems.

Metaphor or literal? I'm thinking metaphor but who can say for sure?
Literal per above.
