Is that the one who was killed by an arrow?

There was also a Thelomen who was around when Iccy trashed the Azath house that became the Treehut.

So unnamed Thelomen, unnamed human taget, Mappo and Veed. Any others (that have been identified - odds are good there are a bunch never ref'd) that i've missed?

Yes, I meant the one mistakenly killed by an arrow. Which provoked Icarium to lay waste to the town the archer had been trying to keep them away from.

No other guardians come to mind, though there must have been lots.

sure there have been lots, but thats the past, and in Icariums case the past, is the past. It's what he will do with Veed at his side instead of Mappo that matters.

It's mentioned in DG that at one point armies followed Icarium, not just single Guardians. I just don't remember the exact page (despite finishing a re-read of the book only 2 days ago).
