Rake's Skykeep was in pretty shoddy condition by time MoI rolled around. Tays nearly destroyed the thing and then Rake went and crashed it into a building. Its no wonder why it broke; it was too damaged to keep working, kinda like an TI that sustains too much damage and gets severed from the ritual. Also, wasn't Rake's Skykeep found inside a glacier? I'm reckoning that would've damaged it too (likewise it could've preserved it :)).

It was a mountain though...so it's nothing if not sturdy.

That's part of SE's brilliance. We know next to nothing about Assail and Jacuruku but he tantalizes us so much with little tid bits.

Mappo's sack -

Yes, it has a warren inside of it, so he can stick an unlimited amount of things into it. It also has some sort of specialized recall power, where he can "call up" any item that he has put in there and it will come back to his hand/visible part of the bag. There is an instance (or maybe more) where he talks about shoving some annoying person in the bag, and having some vague feelings of guilt afterwards as he doesn't know exactly what that would do to someone or where they have gone to. Probably wouldn't want to move into a realm that you didn't know how to get out of and didn't even know if it was liveable.

Edit: Kinda reminds me of the trunk in the first couple Discworld books:D

Its mentioned that an entire clan of Imass have been trapped in the warren of Chaos over on Assail. Is it not impossible to think that were dealing with maybe half a dozen Hairlocks? Insane mages that have decided screw the normal warrens, chosen Chaos, enslaved a few cities in a similar way to Pannion and his Domin, and then plays the game of lets see who can cause as much possible damage to whatever idiots get in our way? Theres no reason why the Tyrants and their armies have to actually fight the Imass. Why not just train your all mages in the use of chaos and get them to open portals and fling the Imass into the deepest reaches of Chaos where even they can't survive?
