There were millions of souls within the sword, and most of them were Bad Dudes. How come there weren't major effects/ripples throughout the world from their simultaneous release?

Sure, we got Draconus and Apsalar'a, Toc and Hood. Beyond that, was there a peep from any of the other inhabitants? (I might not be remembering if so).

Might not have been enough time yet to see the full effects.

As Nacht said, a lot of them got consumed by chaos, as it was, to put it lightly, a pretty hazardous situation in the sword for quite a bit there.

The ones who survived probably didn't have plans ready to go after escaping, as most of them had probably been in there for countless years and had possibly even given up hope of freedom. Many would be so mentally and physically broken as to be no longer capable of much, at least in the immediate aftermath. Even the ones in a state to actually do anything would be very disoriented by the sudden change in their situation, especially those who had been in the sword a very long time, as they'd be thrust into a completely different world than the one that had existed before their imprisonment.

Some might behave rashly and feel invincible after being set free, possibly resulting in underestimating the current denizens of the world and getting themselves stupidly killed. Others might feel incredibly lucky to have gotten out and might be hesitant to push their luck by making waves.
