Here are my 5 in no particular order.

The killer with the bad hair in No Country for Old Men.

The scarred up FBI agent who gets physically ill when women swear at him in The Frighteners.

The long haired sword guy who smokes eyeballs in The Crow.

Brick Top in Snatch.

Jean Baptiste Zorg in The Fifth Element.
Can you dig it ?

This was tough and while some semi-recent ones like Ian McKellen/Magneto in X-MEN and Carradine in KILL BILL came to mind, all my real faves were fairly old school...

James Earl Jones/Thulsa Doom in CONAN.

Alan Rickman/Hans Gruber in DIE HARD.

Anthony Hopkins/Hannibal in SILENCE OF THE LAMBS.

Bruce in JAWS.

Christopher Walken/Vincenzo in TRUE ROMANCE.

- Abyss, loves a good bad guy.

I came up with these 5, but it might be revised if I think of someone better:

- Brick Top (Snatch)
- Marsellus Wallace (Pulp Fiction)
- Shlubb&Klump (Sin City)
- Hannibal (Silence of the Lambs)
- Vito Corleone (The Godfather)

Of that colossal wreck, boundless and bare
The lone and level sands stretch far away."

These are the people that I actually wanted to punch in the face I hated their characters so much. Not because they were poorly acted, the opposite actually. The only three I can ever remember getting really physically angry with.

Nurse Ratchet - One Flew Over the Cuckoos Nest

Worm - Rounders

Commodus - Gladiator
So, you're the historian who survived the Chain of Dogs.
Actually, I didn't.

It seems you stand alone.
It was ever thus.

The only truly evil one I can think of is in a TV show (T-Bag in Prison Break).

On deeper thought, though, here goes:

Darth Vader. All time ultimate kick ass bad guy. Not the most evil or hateable, certainly, but so freaking great!
Dr Evil. So much funnier than Austin Powers!
The Joker (Heath Ledger). Just cos he was so good at it...
The Witch King/Ringwraiths in LOTR. I know that now they aren't too frightening but when it came out, I loved them!
Patrick Bateman - American Psycho (played by Christian Bale) he was nasty!

As a side note, would V (V for Vendetta) be considered a villain? Technically not, I know, but he is a kind of terrorist.
Also, Dr. Horrible. :)

Oooh...the difficulty is in narrowing it down to just five! Ok, so in no particular order:

1. Darth Vader
2. Alan Rickman / Hans Gruber (how can he not be on this list?)
3. Ralph Fiennes / Lord Voldemort (come on, he's freaking creepy in the movies!)
4. Christopher Lee / Saruman
5. Anthony Hopkins / Hannibal Lecter

If I had an "honorable mention" or two, they'd go to Heath Ledger's Joker and Agent Smith from the Matrix trilogy.

In no particular order, and unsurprising given that I am late to the party on this one:

1. Heath Ledger - The Joker The Dark Knight
2. James Earl Jones - Darth Vader Star Wars Original Trilogy
3. Alan Rickman - The Sheriff of Nottingham Robin Hood: Prince of Thieves
4. Forest Whitaker - Idi Amin The Last King of Scotland
5. Christopher Walken - The Archangel Gabriel The Prophecy

Honourable mentions:
Dan Aykroyd - Grocer Grosse Pointe Blank
The Predator - Predator
Sir Anthony Hopkins - Hannibal Lecter Silence of the Lambs
Rutger Hauer - Roy Batty Blade Runner
Viggo Mortensen - Lucifer The Prophecy

Trust me, I'm a doctor.

No love for Gary Oldman in Leon? Also Woo-jin in Oldboy is a pretty good bastard. And Hans Landa in Inglourious Basterds.

Perhaps not my favourite favourites but worth a mention all the same.

So many nasty pasties out there, however...

Ralph Fiennes - Amon Goth (Schindler's List)

Ben Kingsley - Don Logan (Sexy Beast)

Michael Madsen - Mr Blonde (Reservoir Dogs)

Kevin Spacey - John Doe (Seven)

Denzel Washington - Alonzo Harris (Training Day)

Honourable mentions must go to ANY film starring Alan Rickman or Gary Oldman where they are playing baddies and Brick Top...., the 'orrible c**t !
Now all the friends that you knew in school they used to be so cool, now they just bore you.
Just look at em' now, already pullin' the plow. So quick to take to grain, like some old mule.

Joseph Wiseman - Dr. No
Alan Rickman - Hans Gruber
Anthony Hopkins - Hannibal Lecter
Ben Stiller - White Goodman (Dodgeball)
Heath Ledger's Joker

Honourable Mentions:
Russel Crowe and Ben Foster - Ben Wade and Charlie Prince (3:10 to Yuma)
Kevin Costner - Murphy (3000 Miles to Graceland)
Alan Ford - Brick Top

And anything were Kevin Spacey is playing the bad guy. Seven, The Usual Suspects, The Men Who Stare at Goats etc.
