After the Malazan army is annihilated by the rebels outside the city of Aren and Coltaine is killed, Duiker looks at Pormqual and thinks, "Thrice denied, High Fist."

What does this mean? What is denied three times?

If I remember correctly, unfortunately it's been too long since I last did a reread, I think Pom has been denied 3 times to ruin/kill Coltaine himself.

I believe Duiker refers to the fact that Pormqual thrice refused to come in Coltaine's aid, as he and the remnants of the 7th could have been saved.
