Has anyone of the "Old Guard" really died?

(keep in mind that I'm yet to re-read HoC)
The basis of the malazan Empire was founded by Kellanved, Dancer, Ultor and so on. The "Old Guard" is believed dead at the beginning of the series, yet...
Kellanved and Dancer ascended and now rule the Shadow realm.
Cartheron Crust is alive and well aboard the Ragstopper last time I checked.
Dasem Ultor is out there somewhere, but I don't quite remember where he re-appeared.

So, has any member of the "Old Guard" actually died?
